In case of medical emergency, it is very helpful if you have a brief medical history including current medications and doses which you can copy and give to first responders. It is helpful to place this in an obvious location so it can be located if you cannot retrieve it yourself. For example, place it in an envelope labelled "For Medical Emergency" and place it on your refrigerator or next to your phone. Download a form for this.
If you use a cell phone, explore the program offered at myrapidmedic.com. This places a medical history written by you on your phone where first responders can easily locate it. There is also a wallet card with an ID number and a phone number where first responders can get the information if your phone can't be located.
